Please be advised, your temperature will be taken at The Pilates Room prior to admittance into each class. For contact tracing purposes, we require this form to be filled out following a body temperature reading. If you participate in more than one class in a single day, you must retake your temperature and fill out this form each time.  Thank you for your patience and cooperation.
